Dr. Lee has his ACA site, designed by Health Sherpa and connected to the Federal MarketPlace. Clients can explore different plans in the comfort of their home and then contact Dr. Lee to assist them in the actual enrollment. There is no fee for this service.

Please have the name of your preferred primary care physician and the clinic name, address, and phone number—also a list of all other healthcare providers that you see and your preferred hospital. A list of all your medications would help. At the bottom of this page is a paper application. You may use it to collect the information you will need when making the application.
